At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care. On the day-to-day, you will: • Conduct physical therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals • Actively participate in IDT meetings to develop participant care plans • Deliver and document interventions as agreed upon in participant care plans • Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity • Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP) • Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment Job requirements include: • Bachelor’s degree in physical therapy; master’s or doctoral degree preferred • Valid New Jersey physical therapy license, or proof of license eligibility • Graduation from a CAPTE/APTA-accredited physical therapy program required • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred We are seeking a Physical Therapist that ideally has outpatient, acute, inpatient, or skilled nursing experience.
